Endonasal Endoscopic Surgery for Tuberculum Sellae Meningiomas in well selected cases

Abstract: Introduction: Tuberculum sellae meningiomas(TSM) account for about 3% of all intracranial meningiomas. Located in the median skull base and surrounded by important neurovascular structures, they are conventially operated by trans cranial approaches(TCA). Endoscopic endonasal approach(EEA) is recently being used for these tumors.
Methods: We operated in the Neurosurgery Department of Monastir Tunisia from january to November 2022, 8 TSM. 4 were operated by EEA and 4 by TCA. The choice of the approach is based on tumor volume, lateral extension and vessel encasement. In this study, we have compared the epidemiology, the clinic, the radiological aspects, the surgical features and the outcome of the two groups.
Results: No differences found on the epidemiological and cilinical aspects. Radiological features showed a tumor more small, purely median without vessel encasement on the EEA group. The quality of resction wasthe same in both groups. Visual improvement was more remarkable and the recovery was more speedy in the EEA.
Conclusion: The EEA is a good alternative for the treatment of TSM in well selected cases.
