Abstract:
Meningiomas are benign extra parenchymal tumors developed at depend of arachnoid villi. They derive their gravity from their location in hyperfunctional areas and histopatological grade. We report a retrospective series of 164 patients operated on intracranial meningioma in the neurosurgical department of the Mustapha BACHA University Hospital Center, between January 2012 and March 2023.
Our series consists of 53 men (32.36%) and 111 women (67.63%), the W/M sex ratio is 2.09, patients are aged from 16 to 80 years with an average age of 52 years, 59.14% are over 50 years. Anatomopathology concluded that there was a predominance of Grade 1 and Grade 2 among women according to the WHO classification, no patients received radiation therapy. Mortality is 3.65%. Tumor recurrence is present in 21 cases (12.80%) which motivated a second intervention with Simpson I type excision. This recurrence is more common in grade II meningiomas after an average follow-up period of 8 years. The predominance of location of intracranial meningiomas is at the level of convexity 70 cases (42.68%) followed by the location at the level of skull basis with 56 cases (34.14%) was observed, this result is consistent with the data of literature.
A multicentric study in northern Europe for cases (n=906) and controls (n=1774) shows that the risk of meningioma in women, under the age of 50 years, increases with the number of pregnancies (birth of 3 or more children) compared to nulliparous women. In our series, we found that 75% of cases have three children before the age of 40 years compared to 11% nullipares. Indeed, meningiomas are tumors sensitive to hormones, the combined action of progesterone and growth factors contribute to the growth of meningioma.
Our experience has clearly shown us that the risk factors for the occurrence of this pathology are female sex, age above 50 years, number of pregnancies greater than three before age of 40 years, taking progestin. However, the variety of clinical presentations and locations, as well as the varying degrees of aggression of the lesions make intracranial meningioma a protein-forming surgical entity, imposing a management discussed case by case, adapted to each patient.
