Abstract:

Introduction : Sacral chordomas are rare malignant bone tumors, representing more than 50% of all sacral tumors. Complete surgical resection is a chalenge and local recurences remain frequent.


Methods : We select a retrospective case series operated for sacral chordomas over 4 years (2019-2022) in the Neurosurgery department of Monastir Tunisia.


Results : We found 5 males with a mean age of 70 years old, presenting sacral pain in all cases with vesico sphincter disorder in almost all cases and without motor deficit. Radiological investigations showed in all cases an osteolytic huge sacral lesion with invasion of soft tissue and marked contrast enhancement. Through a posterior approach, a total resection was obtained in two cases, a subtotal resection in two cases and partial in one case. All patients recieved postoperative radiotherapy. After a mean follow-up of 28 months we found three recurences and one patient was died from lung matastasis.


Conclusion : Sacral chordomas should be managed in a reference center by a multidisciplinary team and surgical approach is tailored to lesion features. Despite therapeutic advances, recurrences and morbidity remain frequent.
